New DARPA RFI: Multiscale Reasoning Across Human Physiology

The Defense Advanced Research Projects Agency (DARPA), through its Biological Technologies Office (BTO), has released RFI DARPA‑SN‑26‑100 on “Multiscale Reasoning for Human Physiology.” The agency is seeking input from organizations that can connect human biology from molecules to clinical outcomes in support of military medicine.

The focus is on prediction, diagnosis, intervention, and recovery in combat casualty care (CCC) and prolonged casualty care (PCC). DARPA wants to understand existing technologies and datasets that integrate omics, imaging, physiological monitoring, and clinical data across molecular, cellular, tissue, organ, whole‑body, and clinical scales.

  • AI and foundation model companies in biology and medicine
  • Teams building mechanistic models, digital twins, or hybrid mechanistic and AI systems
  • Organizations with longitudinal, multimodal human physiology datasets
  • Clinical and operational medicine groups with trauma or military experience

DARPA highlights several key areas under this RFI, including: 

  • Multiscale computational models that connect biological scales and support realtime or nearrealtime reasoning 
  • Multimodal and biological foundation models that integrate diverse data sources 
  • Longitudinal and multimodal human physiology datasets, including critical care and operational medicine data 
  • Tools for forecasting physiology, predicting treatment response, and supporting clinical decision making
  • Militaryrelevant applications such as combat casualty care, CBRNE exposure, shock, traumatic brain injury, infection, and prolonged casualty care 

Response Essentials and Timing

Respondents are asked to briefly describe their technologies and datasets, explain how different scales are integrated, and share any validation, performance metrics, and key gaps they see in multiscale reasoning. DARPA encourages concise, focused submissions in PDF format, with proprietary information clearly marked.
